For people aged 65 and above, you may be entitled to additional benefits through your health insurance plan.

Nowadays it is common for health plans to offer Home Visit Assessment service. This is an added bonus, free of charge for qualified Medicare beneficiaries, that is designed to close the gaps in medical care. This program does not replace the traditional care provided by their family doctor but is an additional layer to preserve the wellness and health of seniors. With the growing demand for health care, it is becoming difficult for doctors to provide the time in a busy practice to fully cater to your healthcare needs. The Home Visit Assessment program is a 360 evaluation, providing a medical and psychosocial assessment on your time, at your space, at your pace. This is a collegial partnership in conjunction with your primary care physician to ensure that all your medical needs are addressed.

Qualified Medicare beneficiaries are entitled to these assessments on an annual basis. They are furnished by licensed medical providers also known as Advance Practice Providers (APP) such as Physician Assistants (PA) and Nurse Practitioners (NP). They are licensed qualified healthcare professionals that provide care and treatment working under the supervision or collaboratively with a doctor. The APP would commute to the senior’s home to provide the assessment, which includes a broad range of medical, social and psychological evaluation.

This includes but not limited to:

  1. Assessment of medical conditions or diagnoses
  2. Conducting medication reconciliation
  3. Depression and anxiety screening
  4. Cognitive assessment
  5. Activities of daily living (ADLS) assessment
  6. Nutritional survey
  7. Social assessment

These are important issues to identify early and intervene so that the appropriate resources are allocated to support and preserve your healthy lifestyle.
